Registration Form

We are now accepting enrollment applications for the 2024-2025 school year.

 

Please download the registration packet found on the 2024-2025 Registration Packet tab on the left.

 

Once the forms have been completed, please bring them to Sue Sossaman Early Childhood Development Center 18655 E. Jacaranda Blvd., Queen Creek 85142 along with a copy of the student's birth certificate, immunization records, proof of residency (as listed in the packet), and a copy of the enrolling parent/guardian's driver's license. Our front office team is ready to accept your packet and get your student enrolled.
